The Short Answer: What a Hair Transplant Actually Does

A hair transplant does not create new hair, and it does not regrow hair that has been permanently lost. What it does is relocate living hair follicles from an area where they exist (the donor area) to an area where they are absent or reduced (the recipient area). The follicles being moved are alive, healthy, and DHT-resistant — they will continue to produce hair in their new location exactly as they did in their original location.

What Happens to a Follicle During and After Transplant

During extraction, the follicular unit (a group of 1-4 hairs sharing a common sebaceous gland and connective tissue) is removed from the donor area with a small circular punch tool in FUE. The follicle is temporarily stored in a nutrient solution while the recipient area is prepared. During implantation, a channel is created and the follicle is placed at the appropriate angle, depth, and direction. The hair shaft typically sheds within 2-4 weeks as the follicle enters a temporary dormant phase. The follicle itself remains alive beneath the scalp surface. New hair production begins at 3-4 months.

What Happens to the Donor Area

The extracted follicles leave small circular healing wounds in the donor area, each approximately 0.8-1.0mm in diameter. These heal and become essentially invisible once donor area hair grows back to normal length. The follicle itself is permanently removed from the donor area — it does not regrow there. This is why donor supply is finite.

Can Miniaturized Follicles Regrow Without Surgery?

In androgenetic alopecia, hair follicles don't typically die suddenly — they miniaturize progressively under the influence of DHT. Miniaturized follicles are still alive and potentially recoverable, particularly in early stages. This is why medications like finasteride and minoxidil can produce regrowth in some patients — they halt or reverse the miniaturization process. Hair transplantation is appropriate when follicles in the recipient area have miniaturized beyond recoverable function.

The Shedding Phase: When It Looks Like It's Not Working

The most common source of patient anxiety post-transplant is the shedding phase at 2-4 weeks. The hair shaft falls, but the follicle remains alive and implanted. After a dormancy of 2-4 months, the follicle resumes active growth and begins producing a new hair shaft. The shed phase is not a failure; it is a normal biological transition.

What Determines How Much Hair You Get Back

  • Graft survival rate: In well-performed procedures this is 90-95%; in poor-quality procedures it may be 60-70%.
  • Graft count: More grafts means more potential hair production, but is limited by donor supply.
  • Natural hair characteristics: Patients with thicker, curlier hair see more visual coverage per graft than those with fine, straight hair.

Understanding these three variables explains why two patients with identical graft counts can have noticeably different-looking results.
