Non-Surgical Option 1: Finasteride (Oral or Topical)
Finasteride is the most evidence-backed non-surgical treatment for androgenetic alopecia. Oral finasteride (1mg daily) halts progression in approximately 83% of users and produces some regrowth in about 66%. It works by blocking DHT, the hormone that causes follicle miniaturization. Results take 6-12 months to be apparent. It requires ongoing daily use — stopping the medication causes loss to resume. Cost: $10-30/month for generic oral finasteride. Best for: patients in early-to-moderate stages of loss (Norwood 1-4) who want to preserve existing hair.
Non-Surgical Option 2: Minoxidil
Minoxidil (topical or low-dose oral) works by extending the hair growth phase and improving scalp blood supply. Topical minoxidil (2-5% solution or foam) has the longest evidence record. Oral minoxidil (1.25-2.5mg daily) is emerging as the preferred option. Results: meaningful density improvement in 40-60% of users, with best results in early-stage loss. Like finasteride, results cease when treatment stops. Cost: $20-40/month. Best used: in combination with finasteride rather than as a standalone treatment.
Non-Surgical Option 3: PRP and Exosome Therapy
Platelet-rich plasma (PRP) injections and exosome therapy are in-clinic treatments that stimulate follicle activity. PRP involves injecting concentrated growth factors from your own blood into the scalp. Evidence: mixed but positive. Exosome therapy shows promising early data. Cost: $1,000–$3,000 per session, typically 3-4 sessions initially followed by maintenance. Best for: early-stage loss as a standalone treatment, or as a complement to surgical transplantation.
When Surgery Is the Clear Choice
Hair transplant surgery is clearly indicated when: non-surgical treatments have been tried and are insufficient; hair loss has advanced to the point where established bald areas exist that medication cannot reverse; the patient has stable, suitable donor density; expectations are realistic; and the patient is at a stable enough life stage to commit to a 12-18 month recovery and result timeline. Surgery produces permanent results that medication cannot.
The Combination Approach: Best Long-Term Outcomes
The evidence consistently shows that combining hair transplant surgery with ongoing medical maintenance therapy produces the best long-term outcomes. The transplant permanently addresses established loss. Finasteride and/or minoxidil preserve remaining native hair that would otherwise continue to thin after the procedure. This combination allows the transplant result to age naturally rather than becoming increasingly isolated as surrounding hair recedes.
